Client Agreement Policy – Terms & Conditions
Scope of Practice
1. I understand that Chere Stevenson of Life Activated Holistic Therapies is not a licensed medical provider or psychologist and that any services offered should not be considered a replacement for the advice and/ or services of medical and/ or suitably qualified allied health practitioners. If you desire a diagnosis for a symptom Chere will recommend you seek professional advice from such practitioners. By law Chere shall not provide a medical diagnosis or recommend discontinuance of medically prescribed treatments.
2. When a client seeks Chere’s help and is accepted for such care, it is essential for both to work towards the same objective. Kinesiology and counselling offer methods of restoring balance to your energy, body and mind as a way of helping you achieve your health and wellbeing goals.
I understand that I must be willing to actively participate in my own healing, and I am willing to be responsible for my own health.
3. Results: I acknowledge and understand that testimonials and reviews are for reference only and my own personal experience may differ. Results vary from person to person and cannot be guaranteed.
Privacy & Confidentiality
1.The provision of Chere’s services entails the collection and recording of personal information relevant to your current situation. This information is an essential part of your assessment and session protocol, with the primary purpose of providing you with quality care.
2. Access: Any information gathered, recorded and filed is available to you upon request.
3. Confidentiality: All personal information gathered shall remain confidential and secure except in the following circumstances. a) Chere Stevenson of Life Activated Holistic Therapies is subpoenaed by a court, or b) Prior approval has been obtained from you to provide a written report to another professional or agency ie. a medical practitioner or solicitor, or to discuss material with another person. ie. parent or teacher, or c) It is determined that I, as a client or my child(ren) are in imminent danger to myself or others and/ or need further medical/ healthcare support.
Cancellation & Rescheduling Policy
Your courtesy when making appointments is greatly appreciated.
1. Cancellation/ Rescheduling: Your appointment time is reserved exclusively for you, and you are financially responsible for the time reserved. You may cancel or reschedule your appointment up to 24 hrs prior to your appointment time without penalty. If you must cancel or reschedule an appointment due to emergency or being taken ill, please notify Chere as soon as possible. It is understood that life can throw the unexpected at us from time to time.
2. Short Notice Cancellation or No/or Late Shows: If a cancellation is made with less than 24hrs notice, and for no-show appointments a full 100% of the fee of the cost of your appointment will be incurred. A Late Show cancellation (more than 15 minutes late without notification) will also occur a full 100% of the fee of the cost of your appointment. Any deposit paid for an Initial appointment will be non-refundable. Full payment for your next session is required before a new session can be rescheduled.
3. Late Show: Arriving late for your scheduled appointment will result in a shorter session. Lateness of more than 15 minutes will result in your appointment being cancelled, unless otherwise agreed upon or appropriate notification is given. Please let Chere know if you are running late and she will do her best to accommodate you, but sessions will still finish at their scheduled time. Chere is not obligated to make up this time due to scheduling constraints.
Please respect that this time has been exclusively set aside for you and there may be other people who need help who would be happy to take your time slot. Arriving at your Initial consult without your paperwork will also affect the length of your session as this must be completed and handed over at the start of, or prior to your first appointment.
Payment Terms
1. All fees are required to be paid prior to or at the end of each session, no exceptions.
2. Cancellation payments: An invoice will be emailed within 1-2 business days of cancellation notification including full banking details for ease of payment. Payment is required within 7 business days. A client will not be given the right to book new appointments until any invoice/ late fees have been settled.
